Please see pdf link below for  player card template,  to use to print out player cards for any tournaments/showcases/Cup matches that you attend that requires player cards.  

 

Once you Print out the attached template (10 cards per sheet) on paper (Avery 5874 hard stock recommended), then reinsert the hard stock into the printer making sure the CJSL logo is on the back when printing out your player cards.  Also all player cards must be laminated.  

 

If you need cards laminated the CJSL office will help you, free of charge,  but you must make an appointment first.
