Documentation Requirements for CJSL & ENYYSA Players

Every player must provide valid proof of date of birth.

A valid proof of date of birth must be uploaded to the player's profile in GotSport. Upload a clear, complete copy of the document. Do not crop or alter it.

Accepted Proof of Date of Birth

Select the appropriate documentation path based on the player's place of birth and the language of the document.

PATH 1

Born in the USA

Upload ONE of the following:
  1. Birth certificate issued by a government agency
  2. U.S. Passport or U.S. Passport Card (photo page)
  3. Birth registration issued by a government agency or board of health
  4. Document issued by U.S. Citizenship & Immigration Services stating date of birth

Examples of Acceptable Documents

Below are sample illustrations of a U.S. Passport and a New York City Birth Certificate. Submit the complete, unaltered document.

Upload the complete passport photo page, showing the player's name, photograph, and date of birth.

NYC Birth Certificate — Sample

Upload the complete government-issued birth certificate. All information must be clearly visible.

Important — Document Quality

The images above are illustrative examples only. Documents submitted to CJSL must be complete, readable, and unaltered. Cropped pages, edited documents, and screenshots are not accepted.

Born Outside the USA

Passport Has English
  1. Upload the passport photo page as is.
  2. No notarization or translation is needed.
PATH 3

Born Outside the USA

No English on the Passport
The proof of DOB must be BOTH:
  1. Notarized
  2. Translated into English
  3. Uploaded to GotSport with the original document
Documents NOT Accepted
  • Hospital, medical center, or religious certificates (such as baptismal)
  • School IDs
  • Photos or screenshots of a document
  • Cropped, edited, or partial pages
  • Documents not in English without a notarized translation
How to Upload in GotSport
  1. Log in and open the player profile.
  2. Go to Documents → New Document.
  3. Choose Birth Certificate, attach the file, and click Save.
  4. Foreign documents: upload the original and the notarized English translation.

Checklist Before You Submit

Confirm the following before uploading the document to GotSport.

Submit for CJSL Player Approval

Once the required proof of date of birth has been uploaded to the player's GotSport profile, club administrators should submit the Player Approval Form for CJSL review.

Weekly Player Approval Deadline

Thursday at 12:00 PM (Noon) for weekend player approvals.

Registrations received after the deadline will be handled the next business day during CJSL office hours.
